Content is coherent
Whether the page's main content reads as intact, intelligible writing - as opposed to something that has visibly gone wrong.
This is the most severe judgment an SEO report makes. A page whose content has collapsed - raw code or database output where copy should be, corrupted characters, leftover placeholder text, or machine-translated gibberish - cannot rank, cannot satisfy a searcher who lands on it, and signals to search engines that the site is unmaintained. No keyword, title, or link improvement matters until this is fixed.
When this is flagged
The report flags this when the main content is broken or unintelligible as prose: raw code, markup, or data dumps in place of content; encoding corruption (garbled or replacement characters); placeholder or lorem-ipsum text left in production; machine-translation output too garbled to follow; or writing so incoherent a reader cannot tell what the page is saying.
How to fix it
Treat this as an incident, not a content tweak. Find what broke - a failed deployment, a template rendering error, a CMS migration, an unreviewed machine translation - restore or rewrite the content, and check whether the same failure affects other pages.
